WISCONSIN (CBS 58) -- Two hospitals in southeast Wisconsin received a 'D' safety grade by a non-profit.
Leapfrog Group looked at errors, accidents, injuries, and infections.
They found of the more than 2,600 hospitals around the country, 32% got an 'A' grade.
Ascension All Saints in Racine and Ascension Saint Francis in Milwaukee underperformed.
