BROOKFIELD, Wis. (CBS 58) -- Brookfield Police are investigating a death at Silverado Memory Care.
A 95-year-old woman died just days after she was pushed from her wheelchair by another resident.
According to court documents, the resident who pushed the woman has dementia and workers at the facility did not see it happen.
State records show that the facility was nined nearly $10,000 in 2016 after failing to properly supervise a resident known to be violent. That resident allegedly hit other residents on multipe occasions.
A spokesman for the facility tells CBS 58,
We are heartbroken by the situation and are reviewing the events internally as well as assisting local officials in their ongoing investigation.
