Public vaccination clinic to be held at Mequon City Hall April 13

MEQUON, Wis. (CBS 58) -- A public vaccine clinic will be held Tuesday, April 13, at Mequon City Hall.

The clinic will run from 7 a.m.- 7 p.m., and will serve those with appointments as well as walk-ins, at 11333 N. Cedarburg Road.

The city says while some first-dose Pfizer vaccines will be available, the majority of vaccines administered will be the first dose of Moderna.

Anyone 16 years of age and over is eligible. Due to limited supply of the Pfizer vaccine, individuals who are 16 and 17 years of age should schedule an appointment in advance. For questions or help with the scheduler, call Midland Health at 262-754-3130.

Individuals should bring a photo ID and insurance, Medicare or Medicaid card if they have one, and a completed Midland Health consent form. Those without insurance are also eligible for the shot.

Vaccines will be provided at no cost, but insurance will be billed for a vaccination administration fee.

You can find Midland Health's consent form and the CDC's pre-vaccination checklist, here
